- Monthly and routine visits by
Physicians, Medication Manager and
behavioral specialist - Supervision of care personnel by
licensed Personal Care Home
Administrator with 20 years of
experience. - Longevity of staff and low turnover
rates - Ongoing staff education and regular
in-services - Initial assessment visit done timely
within 24 hrs of referral - Initial visit with family involvement for
deciding on care arrangements - Initial assessment visit to help with
equipment and supplies needed - Initial assessment visit will be made
onsite at a facility if not home yet - Accommodating client's needs and
preferences with staff to find most
suitable match - Modifying staffing to match client's
needs if warranted - Family continuous involvement with
care planning process - Regular monitoring of clients needs
